This systematic review aims to summarize cognitive reserve (CR) evaluation approaches and to examine the role of seven selected modifiable lifestyle factors (diet, smoking, alcohol consumption, physical activity, cognitive leisure activity, sleep, and meditation) in mitigating the impacts of age- or disease-related brain changes on cognition. Eighteen population-based English empirical studies were included. We summarize the study designs and identify three CR models that were broadly used in these studies, including a residual model assessing lifestyle factors in relation to unexplained variance in cognition after accounting for brain markers, a moderation model testing whether lifestyle factors moderate the relationship between brain status and cognition, and a controlling model examining the associations between lifestyle factors and cognition when controlling for brain measures. We also present the findings for the impact of each lifestyle factor. No studies examined diet, sleep, or meditation, and only two studies focused on smoking and alcohol consumption each. Overall, the studies suggest lifestyle activity factors (physical and cognitive leisure activities) may contribute to CR and attenuate the damaging impact of brain changes on cognition. Standardized measurements of lifestyle factors and CR are needed, and mechanisms underlying CR need to be further addressed as well.  相似文献

IntroductionThe impact of the nasal septum morphology on the severity of obstruction symptoms has not been fully explored.ObjectiveThis study aimed to investigate whether the morphology of the deviated nasal septum assessed by computed tomography may explain nasal obstruction severity.MethodsThe study included 386 patients who were referred to the computed tomography examination of the paranasal sinuses. Patient selection criteria were the absence of facial anomalies, facial trauma, nasal surgery, and sinonasal tumors. Computed tomography images were used to estimate deviated nasal septum prevalence, the prevalence of Mladina's seven types of deviated nasal septum, and to measure the deviated nasal septum angle. Nasal obstruction severity was assessed by the nasal obstruction symptom evaluation, NOSE scale. The relationship between NOSE score, deviated nasal septum morphology, and deviated nasal septum angle was performed by a statistical regression model on the reduced sample of 225 patients.ResultsThe prevalence of deviated nasal septum was 92.7%. Type 7 deviated nasal septum was the most frequent (34.2%) followed by type 5 (26.2%) and type 3 (23.6%). The worst NOSE scores were recorded in the type 2 deviated nasal septum (45.00 ± 28.28). The mean deviated nasal septum angle in patients with nasal obstruction was 8.5° ± 3.24. NOSE scores were not significantly associated with deviated nasal septum types and angles.ConclusionPatients with different types of deviated nasal septum have different NOSE scores. Computed tomography morphology of the deviated nasal septum could not fully explain the severity of nasal obstruction.  相似文献

背景国内外用于评估癌症患者支持性照护需求的量表较多,但有关此类量表质量的标准化评价研究及不同量表间的横向比较研究较为缺乏,也少有研究者对此类量表的测量特性进行系统的整合与评价。目的评价中文版癌症患者支持性照护需求量表的测量学性能及研究的方法学质量。方法2021年4月检索中国知网、万方数据知识服务平台、维普中文科技期刊全文数据库、中国生物医学文献数据库、PubMed、EmBase、Web of Science、CINAHL Complete数据库,获取有关中文版癌症患者支持性照护需求量表测量学性能评价的研究,检索时限均为建库至2021年3月30日。由两位研究者独立筛选文献、提取资料后,采用健康测量工具遴选标准(COSMIN)系统综述指南,在对量表的测量特性及研究的方法学质量进行评价的基础上,综合评定中文版癌症患者支持性照护需求评估量表各测量特性的证据等级,并形成对于量表的最终推荐意见。采用描述分析法对评价结果进行汇总、分析。结果共纳入15项研究,涉及8个中文版癌症患者支持性照护需求评估量表〔癌症患者支持性照护需求简明问卷中文版(SCNS-SF34)、中文版支持性照护需求筛查工具(SCNS-ST9-C)、癌症患者综合需求评估量表(CNAT)、癌症需求简明问卷(CNQ-SF)、中文版癌症患者未满足需求量表(CaSUN-C)、癌症患者未满足需求简明量表(SF-SUNS)、晚期癌症患者需求评估问卷(ACNQ-41)、晚期癌症患者需求评估表简表(ACNQ-29)〕。就量表的测量特性质量而言,除ACNQ-29的内容效度为"未提及"外,其余7个量表的内容效度均为"不确定";除CaSUN-C、SF-SUNS的结构效度为"充分"外,其余6个量表的结构效度均为"不确定";SCNS-SF34、CNQ-SF、CaSUN-C、SF-SUNS的内部一致性为"充分",ACNQ-41的内部一致性为"不充分",其余3个量表的内部一致性为"不确定";CNAT、CNQ-SF、ACNQ-29的假设检验为"未提及",CaSUN-C、SF-SUNS、ACNQ-41的假设检验为"不确定",SCNS-SF34、SCNS-ST9-C的假设检验为"充分";除ACNQ-41的稳定性为"不充分",SCNS-ST9-C、ACNQ-29的稳定性为"未提及"外,其余5个量表的稳定性均为"充分";仅SCNS-SF34的跨文化效度为"充分",其余7个量表的跨文化效度均为"未提及"。8个量表的推荐等级均为B级。结论SCNS-SF34的测量特性得到了最为全面的评价,其具有较好的信效度,且临床应用可行性高,可暂时被推荐使用,但上述结论仍有待更多高质量证据加以支撑。  相似文献

目的探讨认知重构干预对急性心肌梗死(AMI)介入手术患者不良情绪及疾病感知的影响。方法72例AMI介入手术患者随机分为两组各36例,对照组给予常规护理干预,研究组在对照组基础上给予认知重构干预,比较两组的不良情绪、疾病感知以及治疗依从性。结果干预后,研究组的HAMA、HAMD评分均低于对照组,疾病感知各项评分均高于对照组(P<0.05)。研究组的治疗依从性为94.44%,高于对照组的75.00%(P<0.05)。结论认知重构干预对急性心肌梗死介入手术患者的不良情绪及疾病感知均有积极的影响,可提升患者的治疗依从性。  相似文献

目的探讨腹腔镜微创手术对老年腹股沟嵌顿疝全麻术后认知功能的影响。 方法回顾性分析2015年3月至2018年6月,内江市第二人民医院接受治疗的44例腹股沟嵌顿疝老年患者临床资料,根据手术方式不同分为观察组(24例)和对照组(20例)。对照组采用开放式无张力疝修补术治疗,观察组采用腹腔镜经腹腹膜前疝修补术(TAPP)治疗。比较2组患者手术相关指标、手术前后焦虑、抑郁状态及认知功能及2组术后并发症发生情况。 结果与对照组比较,观察组术后排气时间、进食时间及自主下床活动时间均明显提前,差异有统计学意义(P<0.05)。与术前比较,术后2周2组患者焦虑自评量表、抑郁自评量表评分均明显降低,且观察组明显低于对照组,差异有统计学意义(P<0.05)。与术前比较,术后第2天2组患者即刻记忆力、回忆力、定向力、注意力及语言能力评分均明显降低,且观察组明显高于对照组,差异有统计学意义(P<0.05)。观察组总并发症发生率明显低于对照组,差异有统计学意义(P<0.05)。 结论TAPP配合围手术期精心护理可更有效地缓解腹股沟嵌顿疝老年患者不良心理情绪,对患者术后认知功能的影响更小,并发症少,有利于患者术后恢复,建议临床推广使用。  相似文献

Background

Major depressive disorder (MDD) is a multifactorial disease which often coexists with cognitive deficits. Depression-induced cognitive deficits are known to be associated with aberrant reward processing, neurochemical and structural alterations. Recent studies have shown that chronic electrical stimulation of brain reward areas induces a robust antidepressant effect. However, the effects of repeated electrical self-stimulation of lateral hypothalamus - medial forebrain bundle (LH-MFB) on depression-induced cognitive deficits and associated neurochemical and structural alterations in the prefrontal cortex (PFC) are unknown.

Objectives

We investigated the effect of chronic rewarding self-stimulation of LH-MFB in neonatal clomipramine (CLI) model of depression. During adulthood, neonatal CLI and saline administered rats were implanted with bilateral electrodes stereotaxically in the LH-MFB and trained to receive intracranial self-stimulation (ICSS) for 14 days. The rats were tested for depressive-like behaviors, learning and memory followed by estimation of PFC volumes, levels of monoamines and its metabolites in the PFC.

Results

We found that chronic ICSS of LH-MFB reverses CLI-induced behavioral despair and anhedonia. Interestingly, self-stimulation normalizes the impaired novel object and location recognition memory in CLI rats. The amelioration of learning impairments in CLI rats was associated with the reversal of volume loss and restoration of monoamine metabolism in the PFC.

Conclusion

We demonstrated that repeated intracranial self-stimulation of LH-MFB ameliorates CLI-induced learning deficits, reverses altered monoamine metabolism and the atrophy of PFC. Our results support the hypothesis that chronic brain stimulation rewarding experience might be evolved as a potential treatment strategy for reversal of learning deficits in depression and associated disorders.  相似文献

ObjectiveFunctional processes in the brain are segregated in both the spatial and spectral domain. Motivated by findings reported at the cortical level in healthy participants we test the hypothesis in the basal ganglia of Parkinson’s disease patients that lower frequency beta band activity relates to motor circuits associated with the upper limb and higher beta frequencies with lower limb movements.MethodsWe recorded local field potentials (LFPs) from the subthalamic nucleus using segmented “directional” DBS leads, during which patients performed repetitive upper and lower limb movements. Movement-related spectral changes in the beta and gamma frequency-ranges and their spatial distributions were compared between limbs.ResultsWe found that the beta desynchronization during leg movements is characterised by a strikingly greater involvement of higher beta frequencies (24–31 Hz), regardless of whether this was contralateral or ipsilateral to the limb moved. The spatial distribution of limb-specific movement-related changes was evident at higher gamma frequencies.ConclusionLimb processing in the basal ganglia is differentially organised in the spectral and spatial domain and can be captured by directional DBS leads.SignificanceThese findings may help to refine the use of the subthalamic LFPs as a control signal for adaptive DBS and neuroprosthetic devices.  相似文献
